After services begin, an RN completes supervised visits every 30 to 45 days to review the care plan, monitor for changes, and help ensure support remains appropriate over time. If a client is hospitalized and returns home, a registered nurse also completes a resumption-of-care visit to reassess needs and update the care plan when necessary.
